Job Hazard Analysis and Incident Investigation

How to break a job into steps, find and rank hazards, choose controls, run pre-task planning, report near misses and investigate incidents to find root causes instead of blame.

Most injuries come from hazards that someone could have seen coming. A job hazard analysis (JHA) is the simple, practical tool for seeing them first, and an incident investigation is how a crew learns from the ones that still happen. This course teaches you to choose jobs for a JHA, break them into steps, identify hazards in each step, rank risk, apply the hierarchy of controls, write a clear JHA and turn it into daily pre-task planning and toolbox talks. You will learn what to report, how near misses are the cheapest lessons, how to preserve a scene, interview witnesses without blame, use methods such as 5 Whys and cause-and-effect trees to find root causes, write corrective actions that are tracked to completion, and understand OSHA recordkeeping and reporting basics for serious injuries. It is an awareness-level course based on OSHA and common industry practice and does not replace your employer's program or legal advice.
